OPEN ONLY TO LEGAL RESIDENTS OF CONTIGUOUS US AND DC 21 OR OLDER AT TIME OF ENTRY

1. TO ENTER: Goody's "Gets You Back on Track Fast Sweepstakes" ("Sweepstakes") starts on 7/1/08 at 12:00 A.M., ET and ends on 12/31/08 at 11:59:59 P.M., ET. There are three (3) ways to enter, either: (A) On-line: Go to: www.goodyspowder.com and follow the instructions to submit your on-line entry; (B) Official Entry Form: Legibly complete an Official Entry Form, available at participating retailers, and mail it in a properly stamped envelope to: Gets You Back on Track Fast Sweepstakes, P.O. Box 4104, Grand Rapids, MN 55745-4104; or (C) Mail-In 3"x 5" Entry Card: You may also enter by legibly hand printing your name, complete address, evening phone number and date of birth on a 3"x5" card and mailing it in a properly stamped envelope to the address listed in (B) above. NOTE: On-line entries are limited to one per person per day regardless of e-mail address; however, you may enter as often as you wish by mail. Each entry must be mailed in a separate, properly stamped envelope. Regardless of whether you enter via (B) or (C) above, no mechanically reproduced entries are allowed. On-line entries must be submitted by 11:59 P.M., ET, 12/31/08; mail-in entries must be postmarked by 12/31/08 and received by 1/10/09. All entries become property of GlaxoSmithKline ("Sponsor") and will not be returned. Sponsor is not responsible for incomplete, lost, damaged, stolen, misdirected, late, illegible, or mutilated entries or mail.
2. RANDOM DRAWINGS: This Sweepstakes will concurrently run on the Internet and in various of Sponsor's participating retail accounts. (A) Selection of Semi-Finalists: On or about 1/15/09, the names of ten (10) potential Semi-Finalists will be selected in a random drawing conducted by an independent judging agency, whose decisions shall be final in all matters pertaining to this Sweepstakes. The number of participating retail accounts will determine the number of potential Semi-Finalists that will be selected from among Sponsor's participating retail accounts (one potential Semi-Finalist per participating retail account) and the number of potential Semi-Finalists that will be selected from among all eligible entries received by general mail-in and on-line entries. More specifically, if five of Sponsor's retail accounts elect to participate in the Promotion, five (5) potential Semi-Finalists will be selected from Sponsor's participating retail accounts, one (1) per each of the five (5) participating retail accounts and the other five (5) potential Semi-Finalists will be selected from among all eligible entries received from general mail-in and on-line entries. Similarly, if nine (9) of Sponsor's retail accounts elect to participate in the Promotion, nine (9) potential Semi-Finalists will be selected from Sponsor's participating retail accounts, one (1) per each of the nine (9) participating retail accounts and one (1) potential Semi-Finalist will be selected from among all eligible entries received from general mail-in and on-line entries. A minimum of one (1) potential Semi-Finalist will be selected from among all eligible entries received from general mail-in and on-line entries and a maximum of nine (9) potential Semi-Finalists will be selected from Sponsor's participating retail accounts. Potential Semi-Finalists will be notified by e-mail/phone or mail on or about 1/25/09. Potential Semi-Finalists will be required to sign and return an Affidavit of Eligibility/Compliance (including providing their Social Security Number for tax purposes) and a Liability/Publicity Release, as allowed by state law, by the return date specified in the prize notification letter, or they will be disqualified from the Sweepstakes and, time allowing, an alternate potential Semi-Finalist will be selected. Semi-Finalists will also be issued a Form 1099 as required by law. (B) Selection of Finalist (Grand Prize Winner): On or about 3/29/09, the ten (10) Semi-Finalists will gather in the Goody's Suite at the motor car track in Martinsville, VA to compete for the Grand Prize. Each Semi-Finalist will draw a number from one (1) through ten (10) to determine the order in which the Semi-Finalists will make his/her selection of which one (1) of ten (10) identically sized and weighted wrapped boxes ("Boxes") contains the 'winning' image. The Semi-Finalist who draws the lowest number (i.e., number 1) will make the first selection. All Semi-Finalists will open their selected boxes at the same time and as instructed by Sponsor. The Potential Finalist (Grand Prize Winner) may be required to sign and return an additional Affidavit of Eligibility/Compliance (including providing his/her Social Security Number for tax purposes) and a Liability/Publicity Release, as allowed by state law, by the return date specified in the prize notification letter, or s/he will forfeit the Grand Prize and time allowing, an alternate potential Finalist will be selected. The Finalist will be issued an additional Form 1099 reflecting the approximate retail value of the Grand Prize as required by law. If the Grand Prize Winner declines or forfeits the Grand Prize, Mercury shall conduct a random drawing to select an alternate potential Grand Prize Winner from among the remaining Semi-Finalists. Mercury's decisions will be final in all matters pertaining to this Sweepstakes.
3. PRIZES & ODDS OF WINNING: Grand Prize (1): One (1) Grand Prize Winner will receive a 2008 Dodge® Durango® Limited 4x4 including 20" x 8" aluminum clad wheels, power sunroof, trailer tow group, rear entertainment video system and a 5.7 Liter Hemi® multi displacement system (hereinafter sometimes referred to as "Vehicle") (ARV: $36,226). SPONSOR MAKES NO WARRANTIES, EXPRESSED OR IMPLIED, INCLUDING MERCHANTABILITY OR FITNESS FOR A PARTICULAR PURPOSE, REGARDING THE VEHICLE, INCLUDING ITS CUSTOMIZATION, EXCEPT THOSE CONTAINED IN THE STANDARD MANUFACTURER'S WARRANTY ACCOMPANYING THE VEHICLE. Illustrations, images and depictions of Vehicle are for promotional purposes only, and actual Vehicle received by Grand Prize Winner will differ. All other costs and expenses, including licensing, registration & title fees; delivery charges; collision & liability insurance; emission inspection charges; options/upgrade packages; maintenance and repair; federal, state, and local income taxes; sales and/or use taxes and fees; etc., are sole responsibility of Grand Prize Winner. Grand Prize Winner must provide valid driver's license and proof of insurance upon delivery of Grand Prize and take delivery of Grand Prize at a location designated by Sponsor within four (4) weeks of notification, or Grand Prize will be forfeited in its entirety, and Sponsor shall have no further responsibility to Grand Prize Winner. Other restrictions may apply. If, for any reason whatsoever, Grand Prize Winner should decide not to accept the Vehicle, the Grand Prize will be forfeited in its entirety and Sponsor shall have no further responsibility to Grand Prize Winner. ARV of Grand Prize (including $2,750 ARV of Semi-Finalist Prize) is: $38,976. Odds of winning the Grand Prize depend on the number of eligible entries received and a 1:10 chance of being selected as the Grand Prize Winner from among the ten (10) Semi-Finalists. First Prize (10): Ten (10) Semi-Finalists will each receive a trip ("Trip") the weekend of 3/27/09-3/30/09 for two (2) persons to the Goody's Cool Orange 500 ("Race") scheduled for 3/29/09 at the Martinsville Speedway ®, Martinsville, VA. Trip consists of round-trip coach airfare for two persons from major airport nearest Semi-Finalist's residence to Wyndham-Roanoke Airport; 4 days/3 nights hotel accommodations (one (1) standard room/double occupancy); two (2) tickets to the Goody's Hospitality Suite for the Race; and Two Hundred Dollars ($200) spending money. All Trip reservations and accommodations will be arranged by Sponsor. If a Semi-Finalist resides within 250 miles of Martinsville, VA, the Semi-Finalist will receive Two Hundred Fifty Dollars ($250) in lieu of round-trip airfare for two. (ARV: $2,750). Total ARV of all prizes: $63,726.00 All expenses not specified above, including, but not limited to, meals, beverages, mini-bar, souvenirs, gratuities, insurance, personal services, etc., are each Semi-Finalist's sole responsibility. Travel companion may not be a minor unless Semi-Finalist is his/her parent/legal guardian or spouse. Each Semi-Finalist and his/her travel companion must possess photo identification documents required for travel and must travel on same itinerary. Sponsor has no liability if the Race is cancelled or postponed for reasons beyond the control of Sponsor. Odds of winning a First Prize depend on the number of Sponsor's participating retail accounts and the number of eligible entries received. NOTE: Neither the Grand nor First Prize is redeemable for cash; nor is it transferable or substitutable except by Sponsor with a prize of equal or greater value. All federal, state and local taxes are the sole responsibility of the Winner. Limit: One Prize per household address.
4. ELIGIBILITY: Open to all legal residents of the contiguous United States and DC aged 21 OR OLDER AT TIME OF ENTRY, except officers, directors, and employees (and members of their household or immediate family, i.e., parents, children, spouse and/or siblings and persons residing in such immediate family members' household) of Sponsor; its parent company; its subsidiaries, affiliates, distributors, sales representatives, advertising and promotion agencies and/or their affiliates, assigns or representatives; or any other individuals or entities associated with the development, administration, promotion, execution of, or supply to this Sweepstakes (hereinafter referred to as "Sponsor & Others").
5. INTERNET/LIMITATIONS OF LIABILITY: Sponsor & Others are not responsible for interrupted or unavailable network server or other connections; for miscommunications; failed telephone or computer transmissions; for jumbled, scrambled or misdirected entries or transmissions; for phone, electrical, network, computer hardware or software or program malfunctions, failures or difficulties; for other errors, omissions, interruptions, or deletions of any kind, whether human, mechanical or electronic; or for any damage to any person's computer related to participating in the promotion. Sponsor & Others are not responsible for illegible, unintelligible, late, lost or stolen entries; for incorrect or inaccurate entry information, whether caused by Internet users or by any of the equipment or programming associated with or utilized in the Sweepstakes; or for any technical or human errors which may occur in the processing of any entries in this Sweepstakes. Persons found tampering with or abusing any aspect of this Sweepstakes, as solely determined by Sponsor, will be disqualified and may be subject to prosecution. Any person attempting to enter using multiple e-mail addresses, multiple identities or any other device or artifice to enter multiple times or to interfere with the proper play of this Sweepstakes will be disqualified from participation in the Sweepstakes. If in the judgment of Sponsor, the Sweepstakes is compromised by virus, bugs, non-authorized human intervention or other causes beyond the control of Sponsor, which corrupts the administration, security, fairness or proper play of the Sweepstakes, Sponsor reserves the right, in its sole discretion, to discontinue, suspend or terminate the Sweepstakes. In the event of any such discontinuation, suspension or termination, the winners of any remaining Prizes not yet won will be determined from among all eligible entries received to date, in a random drawing to be conducted following the discontinuation, suspension or termination. Any personally identifiable information collected during an entrant's participation in this promotion will be collected by Sponsor and used by Sponsor, its affiliates, agents and marketers for purposes of the proper administration and fulfillment of the Sweepstakes as described in these Official Rules and in accordance with Sponsor's Privacy Policy as stated at http://consumer.gsk-chus.com/legal/privacy.aspx. CAUTION: ANY ATTEMPT TO DELIBERATELY DAMAGE ANY WEB SITE OR UNDERMINE THE LEGITIMATE OPERATION OF THE SWEEPSTAKES IS A VIOLATION OF CRIMINAL AND CIVIL LAWS. SHOULD SUCH AN ATTEMPT BE MADE, THE SPONSOR RESERVES THE RIGHT TO SEEK DAMAGES OR OTHER REMEDIES FROM ANY SUCH PERSON(S) RESPONSIBLE FOR THE ATTEMPT TO THE FULLEST EXTENT PERMITTED BY LAW. In the event of a dispute as to the identity of a Winner based on an e-mail address, the winning entry will be declared made by the authorized account holder of the e-mail address submitted at time of entry. "Authorized account holder" is defined as the natural person who is assigned to an e-mail address by an Internet access provider, on-line service provider or other organization (e.g., business, educational institution, etc.) that is responsible for assigning e-mail addresses for the domain associated with the submitted e-mail address.
6. RELEASE: Sponsor & Others are not responsible for, whether directly or indirectly, any liability in connection with any injuries, damages or losses of any kind, whether to property or person, including death, arising out of the acceptance, enjoyment, use and/or misuse of any Prize or component of any Prize and/or participation in this Sweepstakes, and they will be held harmless against any such claims of liability arising directly or indirectly from the acceptance or use/misuse of the Prize, travel, overnight accommodations; and/or attendance at and participation in Sweepstakes-related events, including activities that are not sponsored or provided by Sponsor.
7. GENERAL: By entering the Sweepstakes, entrants agree to be bound by these Official Rules. By accepting Prize, each Winner grants Sponsor the right to use his/her individual name, address, photograph, and likeness for advertising, publicity & promotion purposes and for any other such purposes as Sponsor so determines, in any and all media worldwide without limitation, without further compensation, permission or notice, except where prohibited by law. The interpretation of these Official Rules shall be governed by the laws of the Commonwealth of Pennsylvania. Any and all disputes, claims and causes of action arising out of, or connected with, the Sweepstakes or any Prize awarded shall be resolved individually and without resort to class action, in the appropriate courts in the Commonwealth of Pennsylvania exclusively. All issues and questions concerning the construction, validity, interpretation and enforceability of these Official Rules, or a party's rights and obligations, shall be governed by, and construed in accordance with, the laws of the Commonwealth of Pennsylvania, without giving effect to any choice of law or conflict of law rules of any other jurisdiction. All federal, state and local laws and regulations apply. Void in Puerto Rico and where prohibited or restricted by law.
